We are expanding nationally and inviting experienced Support Coordinators and Specialist Support Coordinators who already manage an existing client base to partner with us under a structured 70% fee-share model. Applicants with existing NDIS clients or strong networks in full-service NDIS organisations across Australia are particularly welcome.
Key Responsibilities
Collaborate with NDIS participants and their support networks to identify goals, develop tailored support plans, and link participants with suitable providers to achieve positive outcomes.
Foster participant engagement by building trust and encouraging active involvement in decision-making in their own support planning and implementation.
Manage appointments, coordinate support services, allocate resources, and maintain accurate documentation with regular progress updates to ensure participants stay aligned with their goals and budgets.
Advocate for participant rights and empower participants by educating and supporting them to self- advocate.
Provide proactive support and rapid coordination during times of crisis or significant change to ensure uninterrupted, high-quality service delivery.
Facilitate communication and collaboration between participants, Support Workers, and providers for seamless service coordination.
Monitor service delivery and quality to ensure compliance with NDIS standards and policies.
Contribute to team development by mentoring members and engaging in supervision sessions for professional growth
Qualifications & Requirements
Educational Credentials: Minimum Certificate IV in Disability, or tertiary qualification in Social Work, Psychology, Occupational Therapy, Nursing or related field. (Relevant NDIS registration preferred.)
Experience: Demonstrated experience in NDIS support coordination (Level 2 or 3), with at least 1–2 years working in a full-service NDIS organisation or similar capacity-building role. Experience managing complex or high-needs cases is advantageous. Existing client base or referral networks will be highly regarded.
Skills: Excellent communication, organisation, and time-management skills. Ability to work independently and liaise with diverse stakeholders. High standards of documentation and reporting (NDIS plan reviews, progress notes) are essential.
Other: Must be an Australian resident with the right to work. Reliable internet/phone for remote work. Current Driver’s Licence and own vehicle for participant travel is highly desirable (travel expenses reimbursed by NDIS support items)
